This does math on the sampled color. In order to do that math in the
destination color space, I split the behavior up - the threshold FP
now operates on the input color, and we construct a series with a
simple texture effect (possibly wrapped in a color xform), then the
threshold.
I also added a GM that verifies this behavior. All other GMs using
this effect were operating on a layer source, which is always
created in the destination color space. The new GM explicitly makes
a DAG with an image source, so the alpha threshold filter needs to
handle any color space mismatch.

Auto-allocated IDs mean that the IDs depend upon the order in which
classes happen to get initialized and are therefore not consistent
from run to run. This change paves the way for a persistent shader
cache by fixing the IDs in an enum.

This completely replaces flex with a new in-house lexical analyzer generator,
which we have done for performance and memory usage reasons. Flex requires us
to copy strings every time we need the text of a token, whereas this new lexer
allows us to handle strings as a (non-null-terminated) pointer and length
everywhere, eliminating most string copies.

On desktop, this saves just over 5% of the time in the SkSL compiler.
As written, the code will now build either way, so it's much easier to
switch back (or even have some platforms use SkString, if that's ever
required).

Now that skslc performs all of the optimizations (and then some) that
GrGLSLExpr is responsible for, it's just extra work for no benefit.

GrProcessor.
GrXferProcessor can no longer use this functionality so it is moved to a new intermediate class inherited by GrFragmentProcessor and GrPrimitiveProcessor.

Nonlinear blending mode is going to (sometimes) require additional
uniforms and shader code for color space transformation.
This change just alters the usage of the helper struct so that we can
hide any new logic (without having to change all the FPs that use it).

Combine texture provider and resource provider
Largely mechanical. Only three places that were calling createApprox
via texture provider (ie without flags), so that was simple.

(No longer) Blocked on: https://skia-review.googlesource.com/c/8450/ (Remove asTextureRef from SkSpecialImage & update effects accordingly (take 2))
This also (unfortunately) picks up a few straggling effects that didn't have a sk_sp<GrTextureProxy> factory.

This CL also renames SkSpecialImage::asTextureProxy to asTextureProxyRef
This is a reland of: https://skia-review.googlesource.com/c/7995/ (Remove asTextureRef from SkSpecialImage & update effects accordingly)
It should be good to land since https://skia-review.googlesource.com/c/8701/ (Replace SkSpecialImage::makeTightSubset with asImage (take 2)) fixes the Chrome-side issue
