This began as cleaning up SkSpecialImage.h & spiraled out of control from there.

Combine texture provider and resource provider
Largely mechanical. Only three places that were calling createApprox
via texture provider (ie without flags), so that was simple.

This makes GrPaints usable only once. In some places we must make copies in order to issue draws with the same paint state.

This is step one:
- make SkXfermode useless to public clients
- everything they should need is in SkBlendMode.h
Step two:
- remove SkXfermode.h entirely (since skia core will already be using SkXfermodePriv.h)

This CL doesn't try to resolve any of the larger issues. It just moves the computation of the blurred RRect to the gpu and sets up to start using vertex attributes for a nine patch draw (i.e., returning the texture coordinates)
All blurred rrects using the "analytic" path will change slightly with this CL.

Although the 'canOverwriteSrc' flag is/was being used I still believe we're better off re-adding it after the mask-blur refactoring finishes.

The minimal fix here seems to be handling BoxBlur's return value in SkBlurMaskFilter.cpp::GrRRectBlurEffect::Create. We seem to do enough special handling of the fImage field though that always initialializing it may not be a bad idea.

factory/public-constructor for the class. We want to *not* rely on private constructors, and not rely on calling through the inheritance hierarchy for either flattening or unflattening(CreateProc).
Refactoring pattern:
1. guard the existing constructor(readbuffer) with the legacy build-flag
2. If you are a instancable subclass, implement CreateProc(readbuffer) to create a new instances from the buffer params (or return NULL).
If you're a shader subclass
1. You must read/write the local matrix if your class accepts that in its factory/constructor, else ignore it.

This is part of an effort to ensure that all SkPaint effects can only be
allocated on the heap.
This patch makes the constructors of SkMaskFilter and its subclasses non-public
and instead provides factory methods for creating these objects on the heap. We
temporarily keep constructor of publicly visible classes public behind a flag.

Eliminates SkFlattenable{Read,Write}Buffer, promoting SkOrdered{Read,Write}Buffer
a step each in the hierarchy.
What used to be this:
SkFlattenableWriteBuffer -> SkOrderedWriteBuffer
SkFlattenableReadBuffer -> SkOrderedReadBuffer
SkFlattenableReadBuffer -> SkValidatingReadBuffer
is now
SkWriteBuffer
SkReadBuffer -> SkValidatingReadBuffer
Benefits:
- code is simpler, names are less wordy
- the generic SkFlattenableFooBuffer code in SkPaint was incorrect; removed
- write buffers are completely devirtualized, important for record speed
This refactoring was mostly mechanical. You aren't going to find anything
interesting in files with less than 10 lines changed.

compile on 10.6, exclude failing gms
Sorry for making another CL; rebasing ToT on the old one ended up with code in a tangled mess; was easier to start over.

Reason for revert:
Two problems:
1. broke compile on Build-Mac10.6-* bots
2. broke GM expectations on many bots
We'll need to examine the GM impact before re-landing. I can work with you to do this on Tuesday if you like; just IM or email me.
Original issue's description:
> Perform the same analytic blur calculation on the GPU that we do on the CPU. Results in significant performance gains when using Ganesh to render drop shadows in Chrome.

Speed up drawing large blurry round rectangles by converting them to
nine patches.
SkDraw:
Add drawRRect.
SkBitmapDevice:
Call SkDraw::drawRRect instead of converting SkRRect to an SkPath.
SkMaskFilter/SkBlurMaskFilter:
Create a nine patch of a blurred round rect and draw it instead of
drawing the entire thing.
SkPDFDevice:
Override drawRRect to perform the old behavior in
SkBitmapDevice::drawRect.

I made it as simple as possible. The impact seems minimal and it should do what's necessary to make this code secure.
