DefaultInfo used to not be used when old-style and declared providers were
mixed (struct=(custom='key', providers=[DefaultInfo(...)])).
Also when a single declared provider was returned it used to be treated as an
old-style struct.

Almost every target has an OutputGroupProvider. Putting an
("output_groups", value) pair into SkylarkProviders creates an
unneccessary map. This CL removes it.

This behavior - that 'output_groups' is a provider available
on targets and aspects - has been accidental, but people already depend
on it. This CL keeps that behavior, while fixing the bug that

The headers were modified with

And manual edit for not Google owned copyright. Because of the nature of ijar, I did not modified the header of file owned by Alan Donovan.
The list of authors were extracted from the git log. It is missing older Google contributors that can be added on-demand.

The baseline artifacts are part of the instrumented files provider now, and
are strongly tied to the collect_code_coverage flag. It seems to be simpler
to collect them explicitly in the BuildView (which already collects them for
post-processing), than to rely on the output group selection.
