This is to alleviate problems due to the command buffer getting bent out of shape when the current
OpenGL context is switched out from under it (because we ran a test with a native GL context). This,
however is not a full solution. More changes will be required to ensure that after running each
command buffer or native test we bind the null context. This does allow us to take a step in that
direction without breaking anything too badly. Moreover, there is no real benefit to reusing a
GrContextFactory.
Modifies DEF_GPUTEST to take GrContextOptions rather than a factory to use. Tests were already using
their own factories anyway.
In tests that use GrContextFactory the factory instance is moved to the inner loop.
Modifies gpucts and skia_test to not use persistent GrContextFactories.

Auto-allocated IDs mean that the IDs depend upon the order in which
classes happen to get initialized and are therefore not consistent
from run to run. This change paves the way for a persistent shader
cache by fixing the IDs in an enum.

We need this since we have texture generators that draw the base level
but nothing more. Thus we want them to be able to directly draw into
a pre allocated mipped target instead of doing a copy later.

From the bug it looks like a null fragment processors may be getting into the processor set. This CL tries to plug any gaps in our fragmentProcessor handling.
The only real substantive part to this CL is the addition of some "if (!fp) { return nullptr; }" blocks.
Everything else is just to add chokepoints for processor allocation.

Certain systems experience a leak in the GL driver associated with
stencil buffers. Attempts to avoid the leak (while still using stencil
buffers) dind't succeed. This patch adds a GrContextOption
fAvoidStencilBuffers. This disables certain path rendering modes, as
well as stencil based masking/clipping.

The monolithic GrPipeline is moved to a subclass GrLegacyDrawMeshOp.
The pipeline used to record a GrMesh draw in a GrMeshDrawOp must now be passed rather than implicitly using the op's pipeline.

derived classes.
The first phase of deferring GrPipeline creation until flush will apply only to GrDrawOp subclasses that do not derive from GrMeshDrawOp. This change prepares for that by creating separate draw functions on GrRenderTargetContext for GrMeshDrawOp-derived ops. This is temporary and will incrementally be undone as pipeline-creation deferral rolls out to the GrMeshDrawOps in a later phase of this work.

Combine texture provider and resource provider
Largely mechanical. Only three places that were calling createApprox
via texture provider (ie without flags), so that was simple.

This reverts commit 85eb4226a4cd8c10a0e3f3ba2f3a60efbb2dd61b.

Original change's description:
> Start of rewrite of GrFragmentProcessor optimizations.
>
> This adds a replacement for computeInvariantOutput buts does not use it yet. The replacement allows for three types of optimizations:
>
> * known input color -> known output color for GrFP elimination
> * tracking of whether all color processors modulate their input for the "tweak alpha" optimziation
> * opaqueness tracking
>
> This loses some of the generality of computInvariantOutput. It does not track the known output status of individual color components (other than opaque alpha). It does not track whether GrFragmentProcessors read their input color. It doesn't allow a processor that will receive non-constant output to advertise that it produces a constant output. These could probably be added back in the unlikely case that they prove valuable.
>
> Unlike computeInvariantOutput the optimizations are decided at instantiation time and constant colors are expressed as GrColor4f rather than GrColor.

This adds a replacement for computeInvariantOutput buts does not use it yet. The replacement allows for three types of optimizations:
* known input color -> known output color for GrFP elimination
* tracking of whether all color processors modulate their input for the "tweak alpha" optimziation
* opaqueness tracking
