When SkFontMgr_fontconfig and SkFontMgr_FontConfigInterface create a
typeface from data they do not store the default font name and
getFamilyName will return the empty string. All of the code to handle
this properly now exists, it just needs to be hooked up.

All the SkFontMgr factories currently return bare pointers and sometimes
even document the ownership rules. Since such factories can be
implemented by external ports, the ownership rules should be explicit in
order to prevent simple reference counting issues.

This clarifies ownership rules throughout SkFontHost_Mac.cpp, as well as
tidying up various bits of code. Clarifying ownership means replacing
AutoCFRelease with UniqueCFRef which is based on std::unique_ptr. Most of
the cleanup is removing now dead code and properly indenting.

The COLR table format allows for use of the 'current color'.
Currently the code asserts if this is used, but will use a nearby
luminance grey to this color instead if the assert is ignored.
Remove the assert now that this path is known to be used.

The CFURLCopyFileSystemPath & CFURLCopyFileSystemPath methods
respect the "Create Rule" [1] regarding the ownership of the
returned reference. This means that the objects need to be
deallocated explicitly by calling CFRelease.
