We move syntax for universes from `Misctypes` to `Glob_term`. There is
basically no reason that this type is there instead of the proper
file, as witnessed by the diff.
Unfortunately the change is not compatible due to moving a type to a
higher level in the hierarchy, but we expect few problems.
This change plus the related PR (#6515) moving universe declaration to
their proper place make `Misctypes` into basically an empty file save
for introduction patterns.

We follow the suggestions in #402 and turn uses of `Loc.located` in
`vernac` into `CAst.t`. The impact should be low as this change mostly
affects top-level vernaculars.
With this change, we are even closer to automatically map a text
document to its AST in a programmatic way.

The motivations are:
- To reflect the concrete syntax more closely.
- To factorize the different places where "contexts" are internalized:
before this patch, there is a different treatment of `Definition f
'(x,y) := x+y` and `Definition f := fun '(x,y) => x+y`, and a hack
to interpret `Definition f `pat := c : t`. With the patch, the fix
to avoid seeing a variable named `pat` works for both `fun 'x =>
...` and `Definition f 'x := ...`.
The drawbacks are:
- Counterpart to reflecting the concrete syntax more closerly, there
are more redundancies in the syntax. For instance, the case `CLetIn
(na,b,t,c)` can appears also in the form `CProdN (CLocalDef
(na,b,t)::rest,d)` and `CLambdaN (CLocalDef (na,b,t)::rest,d)`.
- Changes in the API, hence adaptation of plugins referring to `constr_expr` needed.

This is for eventually being reused in Ltac messages ("idtac").

This allows to issue a more appropriate message when a notation with a
{ } cannot be defined because of an incompatible level. E.g.:
Notation "{ A } + B" := (sumbool A B) (at level 20).

Now it is a private field, locations are optional.

This is a bit long, but it is to keep a symmetry with constr_expr.

Applications of it were not clear/unproven, it made printers more
complex (as they needed to be functors) and as it lacked examples it
confused some people.
The printers now tag unconditionally, it is up to the backends to
interpreted the tags.
Tagging (and indeed the notion of rich document) should be reworked in
a follow-up patch, so they are in sync, but this is a first step.
Tested, test-suite passes.
Notes:
- We remove the `Richprinter` module. It was only used in the
`annotate` IDE protocol call, its output was identical to the normal
printer (or even inconsistent if taggers were not kept manually in
sync).
- Note that Richpp didn't need a single change. In particular, its
main API entry point `Richpp.rich_pp` is not used by anyone.

With ocaml 4.01, the 'unused open' warning also checks the mli :-)
Beware: some open are reported as useless when compiling with camlp5,
but are necessary for compatibility with camlp4. These open are now
marked with a comment.

were closed (i.e. the only remaining ones are those of printing/parsing).
Meanwhile, a simplified interface is provided in loc.mli.
This also permits to put Pp in Clib, because it does not depend on
CAMLP4/5 anymore.
