By default, we serialize messages to the "rich printing
representation" as it was done in 8.6, this ways clients don't have to
adapt unless they specifically request the new format using option
`--xml_format=Ppcmds`

Previously to this patch, Coq featured to distinct logging paths: the
console legacy one, based on `Pp.std_ppcmds` and Ocaml's `Format`
module, and the `Feedback` one, intended to encapsulate message inside a
more general, GUI-based feedback protocol.
This patch removes the legacy logging path and makes feedback
canonical. Thus, the core of Coq has no dependency on console code
anymore.
Additionally, this patch resolves the duplication of "document" formats
present in the same situation. The original console-based printing code
relied on an opaque datatype `std_ppcmds`, (mostly a reification of
`Format`'s format strings) that could be then rendered to the console.
However, the feedback path couldn't reuse this type due to its opaque
nature. The first versions just embedded rending of `std_ppcmds` to a
string, however in 8.5 a new "rich printing" type, `Richpp.richpp` was
introduced.
The idea for this type was to be serializable, however it brought
several problems: it didn't have proper document manipulation
operations, its format was overly verbose and didn't preserve the full
layout, and it still relied on `Format` for generation, making
client-side rendering difficult.
We thus follow the plan outlined in CEP#9, that is to say, we take a
public and refactored version of `std_ppcmds` as the canonical "document
type", and move feedback to be over there. The toplevel now is
implemented as a feedback listener and has ownership of the console.
`richpp` is now IDE-specific, and only used for legacy rendering. It
could go away in future versions. `std_ppcmds` carries strictly more
information and is friendlier to client-side rendering and display
control.
Thus, the new panorama is:
- `Feedback` has become a very module for event dispatching.
- `Pp` contains a target-independent box-based document format.
It also contains the `Format`-based renderer.
- All console access lives in `toplevel`, with console handlers private
to coqtop.
_NOTE_: After this patch, many printing parameters such as printing
width or depth should be set client-side. This works better IMO,
clients don't need to notify Coq about resizing anywmore. Indeed, for
box-based capable backends such as HTML or LaTeX, the UI can directly
render and let the engine perform the word breaking work.
_NOTE_: Many messages could benefit from new features of the output
format, however we have chosen not to alter them to preserve output.
A Future commits will move console tag handling in `Pp_style` to
`toplevel/`, where it logically belongs.
The only change with regards to printing is that the "Error:" header was
added to console output in several different positions, we have removed
some of this duplication, now error messages should be a bit more
consistent.

There was a pile of dead code inherited from Cameleon just sitting around and
not used at all. This code was introduced in 2003 and never actually used.

This makes the core free from particular protocol choices.
It should help with the ppx serialization project and shrinks clib.cma a
bit.

There is no remaining hook in the preferences. In particular, the
refresh_editor_hook is gone.

This allows a nifty display of the current state of the document through
a dedicated progress bar.
Also closes bug #3764.

lib/interface split into:
- lib/feedback
subscribe-based feedback bus (also used by coqidetop)
- ide/interface
definition of coqide protocol messages
lib/pp
structured info/err/warn messages
lib/serialize split into:
- lib/serialize
generic xml serialization (list, pairs, int, loc, ...)
used by coqide but potentially useful to other interfaces
- ide/xmlprotocol
serialization of protocol messages as in ide/interface
the only drawback is that coqidetop needs -thread
and I had to pass that option to all files in ide/

- tooltips are attached to sentences
- to find the tooltip text one goes back
until the first start of sentence mark,
finds the corresponding sentence and
displays the tooltip
- focused editing does not invalidate tooltips this way,
since the text, its tags and marks is moved around consistently

A new feedback message for globalization infos can be sent by
Coq to Coqide. Coqide stores the information in the proof of
concept module wg_Tooltip, that also sets things up so that
these infos are displayed as a tooltip when the mouse is over
the text they are attached to. These infos are available only
on locked text, and on the text just processed in the case of an
error (on this piece of text, they vanish as the error tag vanishes
as soon as the user edits the text).
wg_Tooltip stocks these infos as lazy string. This is not needed
in the proof of concept, but is necessary to scale up: Coq may not
generate the full piece of info when the message is sent (because of
high computational cost or big size) and just send an id; later on,
when/if the user asks for the piece of info, the gui requests the
info explicitly using the id.

Main victim is analyzed_view :
- some unnecessary methods have been killed (hep_for_keyword for instance)
- some other migrated elsewhere (recenter_input, find_next_occurrence, ...)
- analyzed_view is now split in two : fileops (filename, save, revert, ...)
and coqops (process_next_phrase, ...)
Four new files created:
- Sentence (for tag_on_insert and alii)
- FileOps (ex-first-half of analyzed_view)
- CoqOps (ex-second-half of analyzed_view)
- Session (ex-record viewable_script and functions about it)
Also lots of renaming, trying to be shorter (but still meaningful) and
more uniform

- Avoid using Util which depends on Compat and hence Camlp4
- Instead, a small Minilib module specific to coqide, which
duplicate 5 functions from Util (50 lines)
- some dead code removal
- the coqlib variable is asked to coqtop
- remove obsolete Util.check_for_interrupt
This way, coqide only depends on 3 files outside ide/ :
Coq_config, Flags, Ide_intf. Makefile and ocamlbuild are adapted
accordingly.
TODO: how should we signal coqide error, warnings, etc ?
For the moment, some Printf.eprintf, some failwith.
To uniformize later...

* generalize the use of .mllib to build all cma, not only in plugins/
* the .mllib in plugins/ now mention Bruno's new _mod.ml files
* lots of .cmo enumerations in Makefile.common are removed, since
they are now in .mllib
* the list of .cmo/.cmi can be retreive via a shell script line,
see for instance rule install-library
* Tolink.core_objs and Tolink.ide now contains ocaml _modules_, not
_files_
* a -I option to coqdep_boot allows to control piority of includes
(some files with the same names in kernel and checker ...)
This is quite a lot of changes, you know who to blame / report to
if something breaks.
... and last but not least I've started playing with ocamlbuild.
The myocamlbuild.ml is far from complete now, but it already allows
to build coqtop.{opt, byte} here. See comments at the top of
myocamlbuild.ml, and don't hesitate to contribute, either for completing
or simplifying it !
